Home health is an excellent, cost-effective alternative to hospital or other institutional care. Service is based on the individual’s need as defined by his or her physician’s plan of care and consists of home visits from skilled nurses, therapists, social workers and/or home health aides.

Home health bridges the gap from an inpatient stay with continued skilled services in the comfort of your home. In the event you're not hospitalized and find it is taxing to leave the home, home health can bring rehabilitative care and treatment to you with your physicians order.

People of all ages use home health services. Careage Home Health specializes in the care of older adults. Some examples include:

• Those who are discharged from a hospital or skilled rehab who need additional continued care
• Those who have short-term health needs
• Those who need teaching and training about their new or chronic disease

Medicare covers 100% of home health services. Private insurance may also cover home health services with varying co-pays and deductibles based upon the individual's policy.

Your physician may order home health for skilled nursing care and/or therapy in your home for a specific treatment plan of care. You can ask your physician if home care is right for you.

HHA are licensed to specific counties and contracted with individual private insurance plans. Your healthcare provider can assist you with options in your area.

Careage Home Health’s employees are skilled clinicians that are licensed and registered in the state of WA. All employees are insured, bonded and screened with background checks. In addition, they receive regular company-provided training and competency evaluations.

A patient’s first visit consists of a complete assessment of physical, cognitive, emotional, cultural, and social needs. From this assessment, an individualized plan of care is developed with input from the patient, family and with physician oversight.

Careage Home Health welcomes family involvement in a patient’s care. Medicare guidelines require physician involvement which allows Careage Home Health to have routine contact with the patient's physician acting as the doctor's eyes and ears.

Yes. A patient’s course of treatment and progress is documented at each home health home visit.

Yes. Education and training is an important part of Careage Home Health’s patient care plan. Our clinicians will appropriately include family members in this process.

Under Medicare guidelines, home health services are provided only to those patients who are homebound and not receiving services elsewhere.

Careage Home Health is licensed to provide skilled services for patients residing in Pierce and Thurston Counties.

Careage Home Health can provide skilled services in your home, a retirement community, assisted living community, adult family home, extended stay or loved one's home. However, Careage Home Health cannot provide service at a long-term care facility.
